Welcome to the online registration for the Canucks Autism Network Family Membership Program. By becoming members of the Canucks Autism Network, your family will be able to register and access CAN programs, events and activities offered in your community, as well as resources from the CAN Provincial Resource Centre. There is an annual fee of $25.00 per family that can be paid online by credit card or in person by cash or cheque.
**CAN family membership fees are now considered an eligible expense by the autism funding unit. For further information about accessing funding, please contact your customer service representative at the unit toll-free at 1-877-777-3530.
In order to register your family, please click here to access the online registration program and submit the following:
- Proof of diagnosis for members of family with ASD. Accepted diagnoses to participate in CAN programs include the 5 disorders that fall under the category of Pervasive Developmental Disorders (Autism Spectrum Disorders) including Autism, Rett’s Syndrome, Asperger Syndrome, and PDD:NOS. The proof of diagnosis can be submitted in the form of a letter from a general practitioner, paediatrician, or child psychologist. This should be faxed, mailed, or emailed to the CAN office no more than 10 days after completing the online registration form.
- Current photo(s) of your family members
- CAN General Waiver and Release (please print a copy, complete and submit the original to the CAN office. Only one waiver is required per family).
Once your payment and application forms are processed and all documents are received, your family will be able to register for programs through the CAN website.
Once registered, all CAN families will receive email updates about upcoming programs and activities. If you have “unsubscribed” from this list but would like to be added again, please email info@canucksautism.ca with “Add to Email List” in the subject line.
